چکیده

A 65-year-old man with situs inversus was admitted with jaundice, nausea, and upper abdominal pain. A computed tomography scan confirmed the diagnosis of situs inversus totalis and revealed a large stone in the main bile duct with distal stenosis. The patient and endoscopist were positioned in the usual manner during endoscopic retrograde cholangiopancreatography (ERCP). As a result of the anatomical abnormality, the duodenoscope was rotated through 180° in D2 to enable visualization of the ampulla. After wireguided cannulation, fluoroscopy showed two giant stones (23mm each) in the main bile duct. Mechanical lithotripsy was attempted but was unsuccessful. Three days later, intraductal cholangioscopy using the SpyGlass DS system (Boston Scientific Inc., Marlborough, Massachusetts, USA) and laser lithotripsy were performed successfully. A fully covered, biliary, self-expandable metal stent was placed (▶Video1) across the stenosis in the distal bile duct. Fragmentation of the large stones was noted, and the patient was asymptomatic 6 weeks later. This is the first case report of a patient with complete situs inversus where ERCP, SpyGlass, and laser lithotripsy were used successfully. We wish to highlight that the endoscopist–patient orientation in this case was the same as that adopted in conventional ERCP in patients without anatomical anomalies. Some case reports of successful ERCP in patients with situs inversus have involved modification to the conventional technique [1]. These modifications include changes to the position of the patient prior to or during the procedure, or a different position for the physician, such as the “mirror” technique [2, 3]. Our impression is that the changes to the classical ERCP techniques are not necessary in patients with situs inversus.

Direct cholangioscopy with standard ultra-slim endoscopes for electrohydraulic lithotripsy of an incarcerated large bile duct stone

Ninety percent of patients with intraductal biliary stones are successfully treated with sphincterotomy and subsequent stone extraction. However, technical difficulty increases with stone size and giant stones require fragmenation to facilitate endoscopic removal. For stones too large to be engaged in a basket for mechanical lithotripsy laser and electrohydraulic lithotripsy have been proposed ...

Successful access to the ampulla for endoscopic retrograde cholangiopancreatography in patients with situs inversus totalis: a case report

BACKGROUND Although various endoscopic techniques in situs inversus have been reported, endoscopic retrograde cholangiopancreatography (ERCP) in patients with situs inversus is always challenging even for an experienced endoscopist. We performed ERCP using two different techniques, and compare the merits of each technique. CASE PRESENTATION A 74-year-old woman presented with epigastric pain a...

Therapeutic ERCP in patient with situs inversus totalis and ampullary diverticulum.

Situs Inversus Totalis (SIT) is a rare entity with complete transposition of all viscera, making endoscopic and surgical procedures challenging and complicated. We describe a rare case of a 55 years old man with SIT and ampullary diverticulum presenting with cholangitis and deranged liver function tests due to common bile duct stones. Therapeutic Endoscopic Retrograde Cholangiopancreatography (...

A Rare Case of Dextrocardia with Situs Inversus Totalis in a Patient of Diabetic Mother

Infants of diabetic mothers are significantly at higher risk for major congenital malformations, with cardiovascular anomalies that is the most frequent. In this study, we presented a rare case of dextrocardia and situs inversus totalis (mirror-image dextrocardia) with multiple congenital heart anomalies who was born from a diabetic mother.

Cholangioscopy-directed lithotripsy for a large bile duct stone: the "donut technique".

Conventionally, choledocholithiasis is managed by endoscopic retrograde cholangiopancreatography (ERCP), and it accounts for at least 50% of the approximately 52000 ERCPs performed in the UK each year [1–5]. Bile duct stone clearance traditionally involves the performance of ERCP with biliary sphincterotomy (or sphincteroplasty), followed by the removal of stones from the biliary tree using an ...
